At Community Care Physical Therapy we pride ourselves on providing a straightforward and efficient process.
How to Prepare
Here is how the process will work. You will be asked to complete an intake form before you seeing your therapist. Make sure that the body part that needs to be evaluated is accessible. Here is an example:
If we’re looking at your knee, please wear shorts or bring them in to change here. You also need a prescription from your doctor.
Our Forms can be filled out in advance online. We will send you a secure link to you so you can fill them out in advance and will link directly to your account.
Your Physical Therapist will do the following:
- Perform an evaluation
You can expect that financials will be discussed at this appointment, so you know what your plan of care will cost.
Please allow up to 60 minutes for this first visit.
